PRE-REGISTRATION FORMS

At Youngstown Orthopaedic Associates, we are continually striving to improve your experience when visiting our practice.  To facilitate this, we ask that you complete your required paperwork (which has been mailed to you under separate cover) and return it to our office (5) days before your scheduled appointment; this paperwork includes HIPAA, Medical History Information, Demographics, and our Financial Policy.

The Medical History forms are unique to each Physician.  Please make sure that you choose the correct paperwork associated with the Physician you are scheduled with.

Along with the history information noted above, we also require that you fill out the DEMOGRAPHICS, FINANCIAL POLICY and HIPAA, which is the same for all Physicians.
