Appointments

The physicians at The Orthopaedic Center require a referral from your Primary Care Physician.  For appointment please call 330-758-0577.  Our phone system will prompt you to the appropriate operator to schedule your appointment.

New Patients

If you have not been in our office or seen one of our Physicians, you may want to facilitate the collection of your personal information by downloading and completing the appropriate forms shown in our Patient Forms section of our website. 

Established Patients

If you have not been in our office in the past year, you may want to facilitate the collection of your updated personal information by downloading and completing the appropriate forms.  Please give the completed forms to the receptionist when you arrive at the office. 

Cancellation of Appointments

Youngstown Orthopaedic Associates, Ltd. requires all cancellations be made at least
(24 hours) in advance of the scheduled appointment; in lieu of this we reserve the right to assess a $25.00 cancellation fee.
